Man... that's exactly how I felt as I was standing there dumbfounded. I don't screw around with safety and I always make sure my cans are tight. Now I'll be questioning myself. I'm not sure I've heard of this happening a single time when it WASN'T user error. People either didn't thread down all the way, cross threaded, didn't lock it, or didn't lock it all the way.
No matter how it happened, send it again.... worst case SilencerCo fixes it and you're out the can for a week or two.
(IMO, it's like pulling the trigger before taking off the safety. Shoot enough, it will eventually happen.)

Put in on my wife’s AR with the QD mount. Handed it to a guy that had never shot suppresses. 3rd shot it went about 30’.
No damage to can. No signs of why. Never had the issue since. I pay closer attention when threading it and locking it down.
