Couple Questions About CHL Forms

    My wife and I just completed our CHL class and have our packets to mail in. The instrutor explained everything well untill it got down to the end and then he kind of got in a hurry and didn't give us time to ask specific questions about the forms. So...I have a few questions about the forms.

    On the blue fingerprint card what goes in the fields titled "DPS NO.", "FPC", and "MNU"?


    On the black fingerprint card what goes in the fields titled "OCA", "FBI", and "CONTRIBUTING AGENCY"?

    On the yellow application, I've lived and worked at the locations in section 13 longer than 5 years. Do I still need to list them again in sections 15 and 16?

    Do I have to send in copies of my birth crtificate and/or drivers license?

    Thanks very much for any advice. I went to the DPS website but couldn't find any specific directions on these questions.

    If the instructor rushed and didn't leave you with everything in hand and complete/ready to go and that was the deal, I'd call him/her and have them help you make sure it's all in order.

    It's gonna be a while sitting in Austin so you want t make sure they don't have to send it back for something to be completed or corrected.

    Well, now that I'm done I don't remember exactly what the deal was but I'm confident I have everything together now and have sent it off. Now I just have to wait a couple of months for them to tell me my prints aren't good enough. I've heard they're rejecting a ton of them and requesting better prints and mine are pretty worn due to years of flint knapping. The guy (retired PD) wrote a letter and explained that to DPS but he said he couldn't guarantee they'd pay any attention to his letter. I've heard some PDs do digital prints so if it comes back, I'll give that a try.

    DPS uses a computer to read and classify prints now. If they are not pretty good, it is likely they will get rejected. Hopefully that won't happen.
