I have had my share of fish hook accidents (through the ear, in the hand), but this might be one of the worst possible ones I could imagine:

I was lucky enough to have a hook come loose or a bass while fishing with a weight and plastic worm. The hook flew into my knee skin and then the weight followed behind and ripped it out. Before I could look down at it
