Guy shoots himself, but how?

Came across this and it looks like he had some sort of holster. It's too blurry to tell but it looks to be the second thing he pulls out of his pants. So if it IS the holster, how the hell did this happen? Maybe he pulled something else out...

Makes no sense if he is using a proper holster that guards the trigger.
