IMHO that was not an AD.

This fan failed to take care of his equipment. Plain and simple.

Was he too cheap to purchase a quality holster once this one started wearing out? How could he miss that it was happening???

Luckily he only injured himself and not anyone else due to his lack of care.

This is why I always tell people to do their due diligence and pay attention to detail. Every time you gear up follow the same exact process consistently and set yourself up for success. Check your gear FIRST, bring your gun into the workspace, chamber check, drop the slide, tap the back to insure it's in battery if you'd like, re-holster the same exact way you draw, and if you run into snags anywhere in the process, stop and figure out the problem then correct it. Be consistent and you'll be less likely for Murphy to rear his ugly face. ;) Telling one's self that "this" time doesn't matter and you'll do it right some other time is a good way to put a bullet in your own ass. LOL
