Lieutenant General (3 star) out ranks a Major General (2 star), because when The made Generals in the Army the senior man in the Infantry was a Sgt Major and the senior man in the Horse Cavalry was a Lieutenant hence Major General and Lieutenant General, 2 and 3 stars respectively.
The rank of 5 star General was created in December 1944. There have only been 5 Generals appointed to 5 star since then, with Omar Bradley being the last in 1950.
