Failure to extract cleanly seems to be the big issue although sometimes it will extract fine and not feed. Looks like the round being loaded hits the feed ramp and doesn't feed correctly in that case
Do you know how to remove the extractor from the slide?
It’s not difficult.
Remove it. Thoroughly clean the extractor channel in the slide. Very common for the channel to get gunked up.
Examine the hooked tip of the extractor.
Is the tip chipped or worn away?
Also try using a different magazine. Any 1911 mag will snap in.
I'd start off with a deep cleaning first, and concentrate on the extractor as Moonpie outlined. Clean and oil. As suggested, try out a different magazine. Many failures in semi-auto pistols stem from magazine issues.
