Found it at a gunshop, had to come home with me. It came with the sheath but not the belt attachment. I had one that worked though. Love it.

I work in a semi-cleanroom environment, and am required to wear the gloves... and a smock, hairnet, beard cover, booties over my shoes, and a facemask.

But.... it takes a good enough edge that I suppose I could do some types of surgery with it....
