I searched for images, and I found that one.... and jumped on it...
but equilateral or reuleaux triangles would work... as long as there's a lip or taper (which there is on round manhole covers, or they would fall in the hole laid flat.)

If there is a cover that is 18.9" on each side, and the hole is 18" on each side, but fits into a recess that is 19" on each side, then it ain't going in the hole... it'll hang on 2 edges of the lid near the corners in 2 corners of the hole.
If the cover and hole were both EXACTLY 18" on a side and 1" thick, it still wouldn't fit if you put a corner in the hole first... because the thickness of the cover will keep the corners from fitting all the way to the corners of the hole- but it could slide thru if you put it in flat (barely, would be a friction fit)

My 1991 Dodge Cummins D250 was a real truck. It really went over 240,000 miles before I sold it and bought a used 2015 Ram 1500 Ecodiesel with 149,000 miles on it. Good trucks.Did they ever make a real truck?
